How To Register

When arriving at the Surgery to register, we require you to complete the following forms in able to register you as a patient;

  1. New patient registration form (GMS1) Required. Download form GMS1 here

  3. Opt in / out form. Required. Record Sharing Opt In / Out Form - download here. Additional information available on this link.
  4. Consent to share (optional). Consent to Share - download here.
  5. Registration for online services (optional). Online Services Application - download here. Information document available on this link.
  6. You will also need to provide identification e.g Passport or driving licence, and proof of your address.

Please allow 5 working days for us to process your registration.

New patients registering with the Practice over the age of 5 are entitled to a check-up with the doctor. This involves discussing your past medical history, arranging your medication and contraception, and a simple examination including blood pressure, weight and analysis of your urine, please bring a urine sample with you.

Registration is with the Practice; however, we run a 'Usual Doctor' system for each patient where you will be asked which doctor you would most like to see. Occasionally one doctor's list is full so you may be asked to register with an alternative.

We believe this policy fosters a close relationship, provides better continuity of care and reduces the possibility of both patients and doctors being uncertain about roles and responsibilities.

Of course, there may be times when you will be offered an appointment with another Doctor if the problem is extremely urgent or if your own doctor is away.



Practice Area

We are currently accepting registrations from patients outside of the practice area.

The practice covers an area approximately eight miles surrounding Seaton. This includes: Beer, Colyton, Colyford, Musbury, Shute, Axmouth and Branscombe. Patients looking to register with the practice can do so by obtaining forms from reception or by completing the online registration forms.

We also have a branch site in Beer. Despite limited opening hours, this proves popular with our patients for ease of parking. Should you wish to make an appointment at our branch site please inform reception.
